This question may seem like a catch 22 but realistically travelling in Peru you will not find a hostel or hotel without one of the above options. The hostels I stayed at were pretty good. We chose based on location and reviews from trip advisor. We definitely did find one of the above in each place.
Here are some of the places I stayed at while travelling Peru:
Loki Hostel (Cusco)
Loki is known as a party hostel! They have location all around South America but I stayed at the one located in Cusco. You can expect great American style food, cheap alcohol and themed parties every night. It's an excellent location, very close to one of the main squares. Depending on how much you want to spend you can either share a large dorm (co-ed or same sex) or have a private room. Showers and bathrooms are nearby and shower pressure was DECENT. The best part were the amazing bar staff and excellent food!

1900 Backpackers Hostel (Lima)
Even though this picture displays a very glamorous looking hostel, it was not. We arrived here very late at night and there was a questionable smell coming from the bathroom. The hostel staff were incredible and very helpful however the place was not the cleanest. We definitely enjoyed our stay there but there could have been some improvements. The shower took 10 minutes just to warmup and the breakfast was a pretty pathetic serving of bread rolls. I would suggest staying at the Loki Hostel located in Lima. I'm sure it will be just as great as it was in Cusco!
